Welcome to Mediline. By using our mobile diagnostic services, you acknowledge and agree to the following Terms and Conditions. These terms protect both you and our company while ensuring high-quality service delivery. If you do not agree with these terms, please do not use our services.
- Services Provided
Mediline provides mobile diagnostic services, including but not limited to:
- Ultrasound Imaging (General, OB/GYN, Vascular, Cardiac, MSK, Abdominal, Thyroid, etc.)
- Board-Certified Radiologist Readings for all imaging services
- Blood Tests & Laboratory Diagnostics
- Cardiac Monitoring & ECG Services
- Home and Corporate Health Screenings
All ultrasound imaging is performed by qualified sonographers, and all imaging interpretations are conducted by board-certified radiologists to ensure medical accuracy.
- Eligibility
- You must be at least 18 years old to book an appointment.
- Minors must have parental or guardian consent for services.
- Services are subject to availability and feasibility based on location and medical requirements.
- Booking and Payment
- Appointments must be scheduled in advance via our website, mobile app, or customer service team.
- Full payment is required before or at the time of service, unless otherwise agreed upon.
- We accept all major insurance plans.
- If you do not have insurance, we accept Visa, cash, and other approved payment methods.
- Patients are responsible for confirming insurance coverage before the appointment.
- Any out-of-pocket costs, co-pays, or deductibles must be paid at the time of service.
- Cancellation & Rescheduling Policy
- Cancellations must be made at least 24 hours prior to the scheduled appointment.
- Cancellations made less than 24 hours before the appointment will be subject to a cancellation fee of [Insert Fee] or may not be eligible for a refund.
- If you need to reschedule, you must notify us at least 24 hours in advance, subject to availability.
- No-Show Policy
- If you fail to be present at the agreed location at the scheduled time, it will be considered a no-show, and you will be charged the full service fee with no refund eligibility.
- Service Limitations & Patient Responsibilities
- Mediline does not provide emergency medical services. If you have a medical emergency, please call 911 or visit the nearest emergency room.
- Our diagnostic services are for informational purposes only and do not replace a doctor’s evaluation or treatment plan.
- Patients are responsible for following pre-exam instructions (e.g., fasting for ultrasound, staying hydrated for certain imaging).
- Mediline is not responsible for any health issues resulting from failure to follow instructions provided before testing.
- Privacy & Data Protection
- Mediline complies with HIPAA (Health Insurance Portability and Accountability Act) and other applicable privacy laws to safeguard patient information.
- Your medical records and imaging results are securely stored and used only for diagnostic purposes.
- We do not share personal health information with third parties unless required by law or with your explicit consent.
- Limitation of Liability
- Mediline is not liable for any medical complications that arise after using our diagnostic services.
- Test results should always be reviewed by a licensed healthcare provider, and we are not responsible for medical decisions based on our reports.
- Our liability is strictly limited to the cost of the service provided.
- Refusal of Service
Mediline reserves the right to refuse service in the following cases:
- Incomplete or false patient information
- Unsafe conditions at the service location
- Abusive, inappropriate, or threatening behavior toward staff
- Failure to follow pre-exam instructions that could affect test accuracy
- Insurance & Payment Policy
- Mediline accepts all major insurance plans for covered services.
- It is the patient’s responsibility to verify coverage before the appointment.
- If insurance does not cover the service, the patient is responsible for payment.
- If you do not have insurance, we accept Visa, cash, and other approved payment methods.
- Any disputes regarding insurance claims should be handled directly between the patient and their insurance provider.
- Governing Law & Dispute Resolution
These Terms and Conditions shall be governed by the laws of the State of New York (NY) and New Jersey (NJ). Any disputes arising from the use of our services shall be resolved exclusively in the courts located in New York or New Jersey.
- Changes to Terms
Mediline reserves the right to modify these Terms and Conditions at any time. Continued use of our services after any changes constitutes your acceptance of the updated terms.
